My son has herniated and bulged disks due to automobile accident. He became addicted to narcotic medication that was given to control his pain. Went through detox but doctor wants him to go to methadone clinic to manage his pain. He completed detox 3 weeks ago and has had no narcotic pain pills since, but today started on the methadone program. I am having a hard time understanding this, can anyone give me insight from personal experience?
I took methadone for pain in my hand (I had a flesh eating bacteria in my hand and lost one finger.) They had been giving me large doses of pain meds in the hospital, and I slowly built up a tolerance to them. Methadone is an excellent pain releiver, but it is something to respect also. It is potent and if he already has an addictive personality he should be aware that someone will monitor how much he is taking. If he is going through a clinic, they will only give him what he needs for a set period, to prevent him from abusing it. Remind him not to take it with any other narcotic he may have or sleeping pills and absolutely no alcohol. Evidently he is being given the methadone through a program, because of his previous addiction to the other narcotic meds, and this is a very good idea. The program should only give him small doses and he will not be able to get more than he should be taking. That is probably why his doctor decided to go this route. The methadone program will treat his pain, it is not being used to detox him from the previous narcotics. He needs to also be aware that if he goes to any hospital, he absolutely must tell the doctors and staff that he is on methadone, so they do not give him anything to interfer with his treatment. The amount that any person takes is based on their condition, and their opiate receptors, and cannot be generalized to the whole population. Also a person who has been on an opiate for an extended period might have had the dose elevated over time, while many will start at lower levels.
I have seen wide variations in methadone doses, for pain patients, and was on methadone for pain for an injury myself for several years. When I was on I took 160 mgs per day at the time I stopped. I had a patient who took 540mgs a day with no side effects, and have seen others who take 10 mgs a days and have drowsiness issues. Thre is no right amount, only the amount which is right for you. Once you have been on the medication for a couple of weeks, and the level has stabilized, if it manages your pain, it is the right amount.
I have a friend who would be bedridden and unable to eat due to pain from cancer pain who had a colostomy, urostomy, prostima, kidney surgeries and now has liver pain that's terminal and scar tissue from the multiple cancer surgeries she had. Her pain management doctor has contact with u.s.a. pain management doctors but not forigen ones plus he does not know the laws in other countries.

I know methadone is available in Mexico. The care is also less expensive there. But here is the thing to consider. Health care is less expensive because there is little to no provision for those who are "broke." If her money runs out first, it is doubtful that she would qualify for any care in a foreign country. It is also a very sad reality that a majority of medical expenses are incurred in the final six months of life unless there are provisions for a euthanasia option. To the best of my knowledge, only Washington state allows that option in the US. For all of the political rhetoric, the US still probably has the most compassionate and best end of life care in the world. Dignity is of utmost importance to the end. I don't think that is the answer you were looking for, but it is the most honest one I can give based upon a great deal of work and study in the field of geriatrics. My thoughts and prayers are with you.

I’m finally out in the free world again after an all-too-long stay in the hospital. I’m quite certain that I could have been released after three or four days, but got stuck there for a week and a half largely because of the holidays — the relevant specialist doctors were on vacation, in addition to the pharmacies running on limited hours and limited staff making it difficult to get my frustrating narcotic regulation-crippled prescription set in motion. I have a nice pile of stories to tell and comments to make, but I’m going to sit on the majority of them for a little while because they’re still unfolding and I need to give them more thought to figure out what the “moral of the story” may be, and also I feel I need to respect the privacy of the other people in the stories (primarily my fellow patients) making me vague on some stuff anyway.
