If someone has been taking methadone for nearly 4 years, and has been prescribed Subutex and they keep saying they are afraid to begin the Subutex for fear of acute withdrawals but they had their last dose of methadone 51 hours ago and are experiencing mild withdrawal symptoms, would it be okay to begin the Subutex? They keep referring to the half-life of methadone and have a fear of not waiting long enough..any help appreciated.
if they wait until they cant bare it anymore then they should be ok.. Most important is what their stable dose of methadone was before they stopped.. they should have been taking 30 mgs or less daily before they stopped, if so then they could take the subutex now... if not, then i would recommend they get some methadone, and get down to a stable dose of 30 mgs or less before stopping then wait 24-48 hours and switch to sub.. if they cant get methadone, then wait until you cant bare it anylonger and take the sub

I am on methadone and have severe constipation. The clinic nurse said I would have to significantly reduce the dose (70 - 80% over time) to get relief. I understand that it is long acting and builds up in your system but is that accurate? There would be no relief after going down 15-20 mg? Thanks!

I take 120 mg of methadone each day. However, i am about to run out of medicine. I have some hydrocodone. I know that methadone blocks the effects of hydrocodone. I want to know when i could expect the methadone to be out of my system so that the hydrocodone will be effective. Also, is there anything I can do to "help" flush the methadone from my system?
You are going to be EXTREMELY ill if you are stopping methadone cold turkey from 120 mgs. I cannot stress enough that this is VERY VERY dangerous and that it is HIGHLY unlikely that hydrocodone will do more than take the tiniest edge off this illness. Methadone is a much more potent drug than hydro and has a much much longer half life in the body. It can be ten days or more before it leaves your body enough for you to "feel" something else, and by that time you will be so sick you can't see straight. PLEASE don't do this to yourself. Get on a methadone clinic, get stabilized, and don't go back to using.

I hear some clinics dose way to much to have you keep coming back
as someone else had said, it depends very much on what kind of opiate is being abused in the first place. in my practice, ive seen the great results with Suboxone. you can find more information about it at http://www.suboxone.com or ask your health care provider.
but to answer your question more specifically, a single dose of 20-30 mg will keep w/d symptoms under control. this would be on the first day of treatment, as an initial dose.. an additional 5-10 mg may be administered on top of the 20-30 if symptoms are still present after 2 to 4 hrs. and so on and so on until symptoms have ceased.
methadone is a funny drug in that its not like your 'at home' opioids.. as your body becomes more used to the structure and effects of the methadone, your tissues are better at recognizing it and the dose 'holds' for longer. keep that in mind.
all of the dosing after the initial day will depend on what exactly it took to calm w/d symptoms. typical doses are usually between 80-120 mg. the drug is then 'titrated down' or 'tapered down' (whatever youd like to call it) as time goes by, but usually on an average of every 2 days.

the baby can withdraw..... after the baby is out though they will have to treat the baby with methadone to wean the child off...... they normally cry alot if you have ever experienced withdraws imagine how it would feel for a baby to experience that.... I was on heavy narcotics b4 my daughter and i went through methadone treatment during my pregnancy i weaned myself off of it so it would not affect my baby after birth but then again i was not in jail and i was able to get 13 days of take home but missing the dose could affect the baby cause of withdraw
