I am on methadone and have severe constipation. The clinic nurse said I would have to significantly reduce the dose (70 - 80% over time) to get relief. I understand that it is long acting and builds up in your system but is that accurate? There would be no relief after going down 15-20 mg? Thanks!

Yes, one of the main side effects with Suboxone is constipation but this can be eliminated usually with a high fiber diet, lots of water, exercise, and if all else fails a mild stool softener (not a laxative). I was diagnosed with cancer three months ago, and only as of recent has severe pain become an issue. After trying several medications, the only one I found to be beneficial for chronic pain is Methadone. Like all of the other opiates, it causes constipation as well as difficulties urinating. This, in my opinion, is the worst side effect. Does anyone know of any method, medicinally - either prescription or an alternative medication, which would help to increase urine output or ease the inability to urinate? Please be kind. Thank you!
Increase your fluid intake. That'll help with both the infrequent urination and constipation. Naturally, your appetite and thirst will be reduced somewhat while on the methadone, so you'll perhaps need to keep yourself on a schedule (e.g every two hours, go have a medium-sized glass of water).
Opioid drugs tend to relax the muscles associated with elimination, so you could conceivably take some caffeinated beverages to give you some caffeine. This will cause your muscles to respond a little more readily (if only because your metabolism is running a little higher) but the usual caffeinated beverages (coffee, tea, colas) usually have the side effect of being laxatives as well. Colas, especially, are high in sugar and cause you to feel the urge to urinate more readily.
I have been on methadone successfully for 15 years. but constipation from it has resulted in terrible constipation and a partial rectal prolapse. regular laxatives don't work. What can be done?
I have the same problem (from morphine). I recommend a warm water enema, probably at least on a weekly basis if not more. It's fast, dependable and without side effects or surprises.

methadone, or any opiate is constipating, as it slows down your gastro-intestinal track. Yes it can cause stomach probs too, if you are experienceing these, it is no big deal, try taking the methadone with some food if it upsets your stomach
